We are seeking a compassionate Registered Nurse to join a growing home health team in Indianapolis. This role focuses on delivering high-quality patient care, completing clinical documentation, and collaborating with care teams to ensure positive patient outcomes.

Key Responsibilities:

Conduct patient assessments, including start-of-care and recertification visits
Provide skilled nursing care, including wound care and ongoing treatment
Complete accurate and timely clinical documentation and charting
Supervise and support LPNs and home health aides
Ensure compliance with home health regulations and standards
Communicate effectively with patients, families, and interdisciplinary teams
Coordinate and manage individualized care plans to support optimal outcomes

Qualifications:

Active Registered Nurse (RN) license
Minimum of 2 years of home health experience
Experience with OASIS assessments
Strong clinical judgment and organizational skills
Ability to work independently and as part of a team
