Our home health team is seeking a dedicated Registered Nurse (RN) to deliver high-quality, patient-centered care in a home setting. This role offers autonomy in the field while collaborating with a supportive interdisciplinary team to improve patient outcomes.

Key Responsibilities:

Perform start-of-care visits, recertifications, and patient discharges in home settings
Deliver skilled nursing care in accordance with care plans and best practices
Assess patient conditions and develop individualized care plans
Administer medications and treatments as prescribed
Monitor patient progress and maintain accurate documentation
Coordinate care with interdisciplinary team members
Educate patients and families on disease management and health maintenance

Qualifications:

Active Registered Nurse (RN) license in California
Minimum of 1 year of RN experience
Home health experience preferred
Experience with electronic health records
Strong clinical, organizational, and communication skills
BLS certification required; ACLS or PALS preferred
