Overview:
Our team is seeking a Registered Nurse (RN) to provide skilled in-home and assisted living care for primarily Medicare-aged patients in the Clearwater area. This role focuses on patient-centered nursing, education, and chronic disease management in a field-based setting. The position offers autonomy, consistent weekday scheduling, and strong team support.

Key Responsibilities:

Provide skilled nursing care including wound care and medication administration
Manage IV therapies and monitor patient responses
Conduct comprehensive patient assessments and ongoing care management
Educate patients and caregivers on treatment plans and disease management
Coordinate care with physicians and interdisciplinary team members
Deliver care in home and assisted living settings
Maintain accurate and timely clinical documentation

Qualifications:

Active Registered Nurse (RN) license in Florida
Minimum of 1 year of RN clinical experience required
Strong patient education and assessment skills
Ability to work independently and manage field-based workload
Reliable transportation for patient visits
