Key Responsibilities:

• Provide skilled nursing care and patient assessments
• Develop and manage individualized care plans
• Administer medications and treatments as prescribed
• Educate patients and families on care plans and health management
• Coordinate care with healthcare providers and clinical team members
• Maintain accurate documentation and timely chart completion
• Monitor patient progress and communicate updates as needed
• Follow established clinical and safety standards

Qualifications:

• Current or eligible Registered Nurse (RN) license
• Minimum of 1 year of RN experience
• Home health experience preferred but not required
• Strong communication and organizational skills
• Current BLS certification
• Reliable transportation and valid driver’s license
