Overview:
Our Home Health Agency is seeking a compassionate Registered Nurse to provide high-quality, patient-centered care throughout Palm Springs and Los Angeles, CA. Key Responsibilities:

• Provide skilled nursing care to patients in home health settings
• Assess patient conditions and develop individualized care plans
• Administer medications and monitor treatment progress
• Educate patients and families on care plans and disease management
• Coordinate care with physicians, caregivers, and interdisciplinary teams
• Maintain accurate and timely clinical documentation
• Monitor patient progress and communicate changes in condition promptly

Qualifications:

• Active Registered Nurse (RN) license in California
• Prior home health or clinical nursing experience preferred
• Strong assessment, communication, and patient care skills
• Ability to work independently and manage patient schedules effectively
• Valid driver’s license and reliable transportation preferred
