Clinical Team Manager Responsibilities
- Lead, mentor, and support a multidisciplinary team including RNs, LPNs, CNAs, therapists, and other clinical staff.
- Provide day-to-day clinical oversight to ensure safe, high-quality, patient-centered care.
- Coordinate patient admissions, referrals, scheduling, and care transitions.
- Review and approve plans of care while ensuring regulatory compliance.
- Conduct clinical documentation and OASIS reviews to support quality outcomes.
- Monitor clinical and operational performance metrics and implement quality improvement initiatives.
- Coach and develop clinicians through education, mentorship, and ongoing performance support.
- Participate in hiring, onboarding, staff engagement, and retention efforts.
- Collaborate with physicians and interdisciplinary teams to ensure continuity of care.
- Provide field support and patient visits as operational needs require.
Qualifications
- Active Massachusetts Registered Nurse (RN) license.
- Associate Degree in Nursing required; BSN preferred.
- Minimum of 2 years of Home Health RN experience required.
- Previous leadership, supervisory, preceptor, or team lead experience preferred.
- OASIS experience required.
- Strong leadership, communication, critical thinking, and clinical decision-making skills.
- Valid driver's license, reliable transportation, and current auto insurance.
