Location: Sonora, CA | Pay: $70.00 – $75.00 / hour (W2 Contract) + Mileage Reimbursement
Shift: Day Shift | 8:00 AM – 4:30 PM

Job Overview
As an RN Care Manager, you will manage and direct in-home nursing care for patients in Sonora, CA. You’ll work independently to visit patients, assess their needs, and collaborate with doctors and therapy teams to help patients reach their health goals.
Key Responsibilities
- Conduct initial and ongoing nursing assessments in patients' homes.
- Create, update, and follow personalized plans of care.
- Provide direct skilled nursing care and educate patients and family caregivers.
- Document patient visits, medications, and care updates accurately using EPIC.
- Coordinate with physical therapy, occupational therapy, and other care team members.
Requirements
- RN License: Active, unencumbered California Registered Nurse license.
- EHR System: Hands-on experience with EPIC is required.
- Certifications: Current BLS or CPR certification.
- Driving: Valid Driver’s License (21+ years old), reliable car, and auto insurance.
- Preferred: BSN degree and prior home health/acute care experience.
