Registered Nurse (RN) – Home Health Case Manager
Denton, TX

Overview:
Our home health agency in Denton is seeking a Registered Nurse (RN) Case Manager to provide comprehensive patient assessments, care planning, and coordination for patients in their homes. This full-time role offers a consistent Monday–Friday schedule with strong benefits and on-call incentive pay.

Position Offers:

  • Schedule: Monday–Friday, 8:00 AM–5:00 PM
  • Competitive salary: $89,000 per year (DOE)

  • Paid Time Off (PTO)
  • Mileage reimbursement at $0.70 per mile
  • On-call pay of $250 for every other weekend

Key Responsibilities:

  • Conduct comprehensive patient assessments
  • Develop and execute individualized care plans
  • Coordinate care across the healthcare team
  • Provide patient education
  • Maintain accurate documentation and ensure compliance
  • Perform basic wound care

Qualifications:

  • Active, current Registered Nurse (RN) licensure in Texas
  • Minimum 1 year of Home Health experience, with OASIS and EMR experience
  • Valid driver's license and auto insurance required
