Overview:
Our home health agency in Lewisville is looking for a dedicated Registered Nurse (RN) Case Manager to make a real difference in patients' lives, one home visit at a time. You'll bring comprehensive assessments, personalized care planning, and compassionate coordination directly to the people who need it most, all while enjoying a predictable Monday–Friday schedule and a compensation package designed to reward your work on the road.

Position Offers:

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Competitive salary: $85,000–89,000 per year (DOE)
• On-call pay of $250 for every other weekend
• Mileage reimbursement

Key Responsibilities:

• Conduct comprehensive patient assessments
• Develop and execute individualized care plans
• Coordinate care across the healthcare team
• Provide patient education
• Maintain accurate documentation and ensure compliance
• Perform basic wound care

Qualifications:

• Active, current Registered Nurse (RN) licensure in Texas
• Minimum 1 year of Home Health experience, with OASIS and Kinnser proficiency
• Valid driver's license and auto insurance
