HIRING IMMEDIATELY: Registered Nurse (RN) with home health experience. Salaried Part-Time position working 3 days a week with benefits!
Amazing opportunity for a RN in the Greater Los Angeles Metro area to complete Home Health and Hospice patient visits.
Position Description:
Registered Nurses provide patient care through patient home visits for contract Home Health and Hospice agencies. . RNs are responsible for observing, planning, implementing and evaluating all assigned patient care in collaboration with other disciplines and in accordance with the physician plan of care. RNs are accountable for all patient care provided, and are responsible for adhering to all policies, procedures and standards of nursing practice.
Duties of Field Registered Nurse Include:

Completing an initial home health assessment of patient and family to determine home care needs for both home health and hospice patients.

Providing a complete physical assessment and history of current and previous illness.

Conducting home visits as assigned, assessing the patient's condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered.

Regularly re-evaluating patient nursing needs, as part of nursing visit. Initiating appropriate preventive and rehabilitative nursing procedures.

Administering medications and treatments as prescribed by the physician.

Providing health care instructions to the patient as appropriate per assessment and plan of care.

Identifying discharge planning needs during nursing visits.

Communicating needs to team as part of the care plan development; assists in implementing prior to discharge of the patient.

Assuring proper maintenance of clinical records in compliance with local, state and federal laws.

Providing patient IV and Wound care when necessary
Qualifications:

Must have Oasis Experience
-
Graduate of an accredited nursing school
-
5 years clinical experience
-
Home health experience with basic knowledge of EMR charting systems.

Must have an appropriate California Registered Nurse license in good standing along with Driver's License and auto insurance. Must have reliable transportation for daily patient visits.

Provide your own basic medical screening equipment.
Salary:
No need to worry about per patient commission rates. We provide a generous salary as a part time W-2 employee. Mileage reimbursed at CA state rate.
