Job Description

Location: Traveling from home to home within Bismarck, ND city limits
Shift: Monday - Friday 8am-4:30pm (Full Time Position)

Position Overview:

A Hospice Registered Nurse (RN) plays a vital role in providing compassionate care and support to patients facing terminal illnesses and their families. They are responsible for managing the physical, emotional, and spiritual needs of patients during their end-of-life journey. Working closely with interdisciplinary teams, Hospice RNs ensure the delivery of high-quality palliative care that enhances the comfort and dignity of patients in their final stages of life.

Key Responsibilities:

Patient Assessment and Care Planning:

  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial status.

  • Develop individualized care plans in collaboration with the interdisciplinary team and patient/family input.

  • Continuously evaluate and update care plans to address evolving patient needs and goals.

Symptom Management:

  • Monitor and manage symptoms such as pain, dyspnea, nausea, and anxiety using evidence-based interventions.

  • Administer medications as prescribed and adjust dosages as necessary to maintain optimal symptom control.

  • Provide education to patients and families regarding symptom management techniques and medication regimes.

Emotional Support and Counseling:

  • Offer emotional support and counseling to patients and families facing end-of-life issues.

  • Facilitate open communication and provide guidance on coping mechanisms and advance care planning.

  • Collaborate with social workers, chaplains, and counselors to address psychosocial and spiritual needs.

Coordination of Care:

  • Coordinate care services with other members of the interdisciplinary team, including physicians, social workers, and therapists.

  • Arrange for additional support services such as home health aides, volunteers, and respite care as needed.

  • Serve as a liaison between patients, families, and healthcare providers to ensure continuity of care.

Documentation and Compliance:

  • Maintain accurate and thorough documentation of patient assessments, care plans, and interventions.

  • Adhere to regulatory standards and agency policies regarding documentation, confidentiality, and quality of care.

  • Participate in quality improvement initiatives and compliance audits as required.

End-of-Life Care and Bereavement Support:

  • Provide compassionate end-of-life care that respects patients' wishes and promotes comfort and dignity.

  • Offer bereavement support to families during the dying process and in the immediate aftermath of death.

  • Facilitate access to grief counseling and support groups for family members and caregivers.

Qualifications:

  • Active RN license in the state of North Dakota.

  • Basic Cardiac Life Support (BCLS) Certification

  • Strong assessment, communication, and interpersonal skills.

  • Ability to work collaboratively in a multidisciplinary team environment.

  • Compassion, empathy, and a commitment to providing quality end-of-life care.
