Job Description

Job Type: Permanent, Full Time

Location: Lewisville, TX

Schedule: Monday through Friday

Key Responsibilities:

- Deliver comprehensive nursing care to hospice patients in accordance with the care plan.
- Conduct regular assessments of patients' conditions and adjust care plans as necessary.
- Administer medications and treatments as prescribed by the physician.
- Educate patients and their families about the disease process, care techniques, and end-of-life issues.
- Collaborate with the interdisciplinary team to ensure coordinated and compassionate care.
- Maintain accurate and timely documentation of patient care and progress.
- Participate in on-call rotation as required.
Strong assessment, clinical, and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.

Qualifications:

- Current and unrestricted RN license in the state of Texas.
- Minimum of 1-2 years of nursing experience, preferably in hospice or palliative care.
- Valid driver's license and reliable transportation.
