RN Hospice Case Manager

Job Responsibilities:
- Assess patients and create personalized care plans.
- Give medications and treatments, watching for side effects.
- Work with the care team to coordinate patient care.
- Support and educate patients and families about their health and end-of-life care.
- Check and record vital signs and patient progress.
- Attend case meetings and communicate with the healthcare team.
- Help improve quality of care and follow regulations.
- Guide and support LPNs and other care staff as needed.
- Keep patients’ environment safe, comfortable, and compassionate.

Qualifications:
- Active Registered Nurse (RN) license in good standing with the State of Indiana
- Experience in hospice, palliative, and geriatric end-of-life care preferred
- Strong clinical judgment, critical thinking, and case management skills
- Skilled in symptom management, patient education, and interdisciplinary care coordination
- Reliable transportation and a valid driver’s license for travel between patient locations
