Overview:
We are seeking a compassionate Hospice Registered Nurse to provide skilled, patient-centered care in home and facility settings. This role focuses on supporting patients and families through end-of-life care while ensuring comfort, dignity, and continuity of care. The ideal candidate is experienced, empathetic, and confident working independently within a collaborative hospice team.

Key Responsibilities:

Provide skilled nursing care to hospice patients in home and facility settings
Develop, implement, and manage individualized care plans
Monitor patient conditions and communicate changes to the care team
Educate patients and families on care plans, expectations, and disease progression
Coordinate care with physicians and interdisciplinary hospice staff
Arrange necessary services, referrals, and supportive resources
Maintain accurate, timely clinical documentation
Provide emotional support to patients and their families throughout end-of-life care

Qualifications:

Active Registered Nurse (RN) license in the state of Texas
Minimum 2 years of hospice experience required
Strong clinical assessment and patient care skills
Excellent communication and interpersonal abilities
Ability to work independently in a home-based care setting
Compassionate, patient-centered approach to care
