Job Title: Hospice RN Case Manager

Schedule: Monday – Friday
Guaranteed Hours: 36 Hours Weekly
EMR System: McKesson / Netsmart Preferred
Job Requirements
- Active Vermont RN License required
- Minimum 1 year of experience required
- Hospice case management experience required
- Home hospice care experience required
- Rural travel experience preferred
- Experience with McKesson/Netsmart EMR preferred
- BLS/BCLS required
- Holiday coverage required
- Prefer candidates with no RTO requests during contract

Estimated Traveler Pay WEEKLY GROSS $2,418.00 WEEKLY TAXABLE $864.00 WEEKLY NON-TAXABLE $1,554.00 HOURLY TAXABLE $24.00 HOURLY BLENDED $67.17 OVER TIME $36.00 - Overtime: After 40 hours at 1.5x rate
- Mileage Reimbursement: IRS rate for travel between patients
