We’re hiring a Hospice Case Manager (RN) who wants meaningful one-on-one patient relationships, a flexible part-time schedule, and the opportunity to provide compassionate end-of-life care while maintaining an excellent work-life balance.

You’ll step into a field-based hospice role providing direct patient care in homes and care facilities throughout Easton, Lakeville, Taunton, and Rockland. You’ll independently manage your patient caseload while collaborating with an experienced interdisciplinary team dedicated to providing exceptional end-of-life care.

What makes you a great fit
• You hold an active Massachusetts RN license
• You have at least 1 year of nursing experience
• Hospice, home health, ICU, or oncology experience is preferred
• You value compassionate, patient-centered end-of-life care
• You’re comfortable independently managing a field-based caseload
• You work well as part of an interdisciplinary healthcare team
• You’re comfortable traveling throughout Easton, Lakeville, Taunton, and Rockland

Compensation & Benefits
• $48–$52 per hour
• Part-time schedule: 24 hours per week / 3 days per week
• Weekend on-call coverage: $500 per weekend
• Routine on-call visits: Additional $100 per visit
• On-call admissions: Additional $200 per admission
• Mileage reimbursement: $0.42 per mile
