We’re hiring a Hospice Case Manager (RN) who wants competitive compensation, professional independence, and the opportunity to build meaningful relationships with patients and families while providing compassionate end-of-life care.

You’ll step into a field-based nursing role focused on providing personalized hospice care to patients in their homes throughout Lithonia, Ellenwood, Stone Mountain, and Decatur. With approximately 4 patient visits per day and support from an interdisciplinary hospice team, you’ll have the opportunity to prioritize patient comfort, dignity, and quality of life.

What makes you a great fit
• You hold an active Georgia RN license and current CPR certification
• You have at least 1–2 years of recent home-care nursing experience
• Previous hospice or palliative care experience is preferred
• You’re comfortable performing patient assessments, managing symptoms, and developing individualized care plans
• You have strong communication skills and clinical judgment
• You enjoy educating and supporting patients and their families
• You hold a valid driver’s license and have reliable, insured transportation
• You’re passionate about providing compassionate, patient-centered end-of-life care

Compensation & Benefits
• Starting salary around $92,000 annually, with flexibility based on experience
