Key Responsibilities
Collaborative Care
- Partner with healthcare providers, nutrition services, and behavioral health professionals to deliver comprehensive patient care.
- Support holistic treatment plans to improve patient outcomes.
Care Coordination
- Coordinate healthcare appointments and follow-up services.
- Ensure continuity of care through effective communication with providers and caregivers.
Team Education & Leadership
- Train and support staff in medication administration and delegated nursing tasks.
- Ensure compliance with applicable regulations and organizational policies.
Medical Record Management
- Maintain accurate and up-to-date medical records.
- Review and audit documentation related to health concerns and delegated nursing responsibilities.
Schedule
- Generally Monday through Friday daytime hours.
- On-call rotation approximately every third week.
- On-call responsibilities are primarily phone-based, with rare in-person requirements.
- Strong leadership and clinical support available during on-call periods.

Qualifications
- Active and unrestricted Registered Nurse (RN) license.
- BSN preferred.
- Minimum of one (1) year of clinical nursing experience within the last five years.
- Current state RN license in good standing.
Physical Requirements
- Frequent standing, walking, bending, and reaching.
- Occasional kneeling, climbing, stooping, crouching, and balancing.
- Ability to lift up to 25 lbs independently.
- Ability to push/pull up to 40 lbs of force.
- Ability to safely transfer up to 50 lbs.
Requirements
- Active State RN License
- BLS (if required by facility)
- Excellent clinical judgment and communication skills
- Ability to work in a fast-paced healthcare environment
