Key Responsibilities

Collaborative Care

• Partner with healthcare providers, nutrition services, and behavioral health professionals to deliver comprehensive patient care.
• Support holistic treatment plans to improve patient outcomes.

Care Coordination

• Coordinate healthcare appointments and follow-up services.
• Ensure continuity of care through effective communication with providers and caregivers.

Team Education & Leadership

• Train and support staff in medication administration and delegated nursing tasks.
• Ensure compliance with applicable regulations and organizational policies.

Medical Record Management

• Maintain accurate and up-to-date medical records.
• Review and audit documentation related to health concerns and delegated nursing responsibilities.

Schedule

• Generally Monday through Friday daytime hours.
• On-call rotation approximately every third week.
• On-call responsibilities are primarily phone-based, with rare in-person requirements.
• Strong leadership and clinical support available during on-call periods.

Qualifications

• Active and unrestricted Registered Nurse (RN) license.
• BSN preferred.
• Minimum of one (1) year of clinical nursing experience within the last five years.
• Current state RN license in good standing.

Physical Requirements

• Frequent standing, walking, bending, and reaching.
• Occasional kneeling, climbing, stooping, crouching, and balancing.
• Ability to lift up to 25 lbs independently.
• Ability to push/pull up to 40 lbs of force.
• Ability to safely transfer up to 50 lbs.

Requirements

• Active State RN License
• BLS (if required by facility)
• Excellent clinical judgment and communication skills
• Ability to work in a fast-paced healthcare environment
