(Archived) Registered Nurse Population Health
Job Description
Purpose of Position:
To provide a registered nurse (RN) perspective on an interdisclipinary team to improve health outcomes of the residents within the region through collaboration with the Health Corporation Patient Centered Medical Home (PCMH) and outside agenices; specialize in patient care processes and clinical workflows; assist providers as an RN on village community travel;
Uphold the organization’s vision, mission, and corporate values. Demonstrate understanding of and compliance with organization’s policies, procedures, code of conduct and work rules.
Essential Functions:
• Practice as a clinical registered nurse
• Promote patient independence through helping establish patient care goals and teach patients and their families to understand conditions, medications, and self-care skills
• Facilitate the collaboration between clinic RNs, providers and agencies to provide ongoing follow-up and care coordination services; monitoring patient care plans.
• Participate in the development and validation of electronic health record tools to improve population health goals
• Collaborate with cross-functional teams inlcuding specialty clinics and other events in support of healthcare promotion and care coordination
• Perform other duties as assigned
