Job Description

Purpose of Position:

To provide a registered nurse (RN) perspective on an interdisclipinary team to improve health outcomes of the residents within the region through collaboration with the Health Corporation Patient Centered Medical Home (PCMH) and outside agenices; specialize in patient care processes and clinical workflows; assist providers as an RN on village community travel;

Uphold the organization’s vision, mission, and corporate values. Demonstrate understanding of and compliance with organization’s policies, procedures, code of conduct and work rules.

Essential Functions:

Practice as a clinical registered nurse

Promote patient independence through helping establish patient care goals and teach patients and their families to understand conditions, medications, and self-care skills

Facilitate the collaboration between clinic RNs, providers and agencies to provide ongoing follow-up and care coordination services; monitoring patient care plans.

Participate in the development and validation of electronic health record tools to improve population health goals

Collaborate with cross-functional teams inlcuding specialty clinics and other events in support of healthcare promotion and care coordination

Perform other duties as assigned

Practicing clinical registered nurse

High level knowledge of care coordination processes

Knowledge of chart documentation and electronic health record

Knowledge of referral services and interdisciplinary team leadership

Required Skills and Abilities:

All employees should have patient/customer service skills, interpersonal skills and good communication skills. All employees should have the ability to manage their time and organize their work to ensure efficient performance. In addition, all employees should have the ability to follow policy, procedure and instructions, actively communicate with their supervisors to ensure understanding and manage their work accordingly. Finally, all employees should have the ability to understand and manage patient/customer confidentiality.

Minimum Requirements
Education Associate’s Degree Nursing
Experience 1 year RN experience.
Credentials Alaska RN License, Basic Life Support,
NRP may be required
