Job Type: Registered Nurse – Med–Surg Inpatient and Outpatient
Schedule: Full-Time, Primarily Day Hours with Occasional Evenings or Saturdays

Position Overview

Join a caring nursing team serving a vibrant and diverse community at a Family Health Center. In this role, you'll provide direct and indirect nursing care, engage in patient education, support medication management, and work closely with the Clinical Nurse Manager and healthcare team to deliver culturally competent, high-quality care.

Responsibilities

• Assess basic health status and care needs for patients either by phone or during clinic visits

• Analyze patient data and make informed care decisions, adjusting plans as needed

• Evaluate nursing interventions and initiate care plan changes when appropriate

• Provide both direct and indirect care and deliver patient education

• Manage medications and assist with medication refill coordination

• Collaborate with a diverse healthcare team to cultivate a safe, positive work environment
