We have a position open for a care manager who will conduct comprehensive member psychosocial assessments and develop care management treatment plans for members with high level acuity needs, particularly those with severe mental illness and high rates of behavioral health utilization and/or severe psychosocial vulnerability. The position requires the candidate to reside in Polk County for field visits to members.
The essential functions of the role include assessing, planning, implementing, monitoring, and evaluating options and services to meet the complex psychosocial health needs of inpatient and outpatient members, managing a caseload in compliance with contractual obligations and WellCare standards, and acting as a liaison and member advocate between the member/family, physician and facilities/agencies.
The candidate must have a Master's Degree in Social Work, Psychology, Counseling, Rehabilitation, or other relevant field, along with 2+ years of experience in behavioral health management and/or acute behavioral health care setting focusing on outpatient/inpatient utilization, case management, and discharge planning. The candidate should also have 1+ year of experience in current case management as a licensed professional and working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs.
The candidate must have a license as required of the position, including a Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), or Licensed Clinical Marriage & Family Therapist (LCMFT). The candidate must also reside in Polk County for field visits to members.
The position may require extensive travel to member's inpatient bedside, home or other facility, particularly for those engaged in Florida's SMI program.
