About the Position:
The Advanced Practice Registered Nurse collaborates with a team of experienced social workers, clinicians, case managers, and other professionals to deliver comprehensive care to adults facing severe mental illness. In this role, the APRN establishes therapeutic alliances to support clients in achieving treatment objectives. Demonstrating adeptness in independent clinical decision-making while seeking appropriate guidance from colleagues, the APRN is responsible for providing psychiatric care and medication-assisted treatment (MAT) for clients with co-occurring mental health and substance use disorders.
Key Responsibilities:
- Perform comprehensive clinical assessments and reassessments for assigned clients, as required.
- Create personalized treatment plans based on clinical evaluations.
- Conduct medication evaluations, manage medications, and provide medical consultations with ongoing monitoring in collaboration with primary care providers.
- Provide on-call support when necessary.
- Take a leadership role within an interdisciplinary treatment team to ensure cohesive treatment planning and evaluation for assigned clients.
- Communicate changes in treatment plans, medical requirements, and service needs to all relevant staff.
- Ensure adherence to all policies, procedures, and protocols related to psychiatric services delivery.
- Maintain accurate documentation of clients' medical and psychological histories, assessments, diagnoses, treatment plans, and outcomes in the Electronic Medical Record system within specified timelines.
- Disseminate essential operational policies and procedural updates to Agency personnel as required.
- Cultivate and maintain relationships and partnerships between the Agency and external organizations.
- Promote a culture of excellence, continuous improvement, and a dedication to quality.
- Participate in mandatory program meetings and relevant training sessions.
- Adhere to all agency policies, codes of conduct, and ethical standards.
- Understand and comply with agency health and safety policies, including participating in drills and attending related training sessions.
Qualifications:
- Master's Degree in Nursing and Nurse Practitioner certification.
- Possession of Psychiatric certification or actively pursuing certification in the field.
- Current and unrestricted APRN license in the State of Vermont (or ability to obtain one upon hire).
- Proven experience in offering psychiatric care and medication-assisted treatment for adults with severe mental illness and co-occurring substance use disorders.
- Background in working with individuals dealing with severe mental illness, substance use disorders, developmental disabilities, or co-occurring medical conditions.
- Up-to-date knowledge of psychiatric medications.
- Proficient in computer usage, including Electronic Medical Record systems.
- Demonstrated compassion, sensitivity, and sound decision-making skills.
- Valid driver's license, reliable transportation with personal automobile liability insurance meeting State of Vermont requirements.
