Registered Nurse (RN) - Behavorial Health Care (Primary Care) Job Overview:

  • Member of the Behavioral Health team, serves as the Behavioral Health Care Facilitator (BHCF).
  • Provides care facilitation services for patients prescribed psychotropic medications by their Primary Care Manager (PCM) within the primary care environment.
  • Primarily includes the conditions of depression, anxiety disorders, and PTSD.
  • Monitors adherence to treatments, provides education, coordinates care, and maintains data registries in the execution of primary care behavioral health services.
  • May assist the PCM and PCMH in completing a number of nursing tasks, it is imperative that the BHCF is aware of his or her primary responsibility: to serve as a liaison between the patient and the PCM; and to facilitate care for patients newly prescribed long-term and/or daily medications (by their PCM) for anxiety, depression, or PTSD.

Registered Nurse (RN) - Behavorial Health Care (Primary Care) Job Qualifications:

  • BLS
  • Computer Competency
  • RN Licensure: Current, full, active, and unrestricted license. Any U.S.
  • Associates Degree of Nursing (ADN)
  • Graduate from a NLNAC or CCNE accredited college or university
  • Certified/Eligible by ANCC as a Psychiatric and Mental Health Nurse
  • 36 months of total nursing experience in a direct patient care clinical setting
  • 24 months of full time experience in mental health.
  • 24 recent consecutive months of direct patient specialty care experience providing services such as psychiatric nursing or behavioral health and wellness counseling.

  • Working knowledge and a minimum of 36 months of experience with levels of care to include post-hospital and acute care illness stabilization. Must possess ability to anticipate and articulate to health management and/or interdisciplinary team the patient's needs, requirements, abilities, and limitations.
  • A minimum of 36 months of experience with professional nursing care standards, theories, principles, practices, disease processes and procedures to perform nursing assignments of moderate to severe difficulty and experience with acutely and chronically ill adult and geriatric patients as appropriate.
  • Must possess a working knowledge of human growth and development, and pathophysiology of disease processes, as they apply to the care environment for all ages of the spectrum from pediatrics to geriatrics.
  • A minimum of 36 months of experience with a variety of psychopharmacological agents used in patient treatment, the desired effects to include side effects and complications of their use, as well as the accurate administration of the psychopharmacologic agent. Must possess a working knowledge of the administrative requirements for proper documentation of patient conditions including disease progress, acknowledgement of teaching, and follow up care.
  • Must have the ability to establish a nursing plan of care that effectively applies core disease management principles to mental health conditions and includes in that plan: a) assessment and identification of target populations; b) implementation and execution of Clinical Practice Guidelines or standards; c) collaboration and coordination of activities with other healthcare members; d) promotion and education of patient self-management actions.
  • Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous.
  • Must possess organizational, problem-solving, and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.
  • 12 months of direct hands-on experience in the last 24 months with computer operations, basic word processing, data entry, and use of an automated medical record system.
  • Must have working knowledge of the military health care system including TRICARE health benefits is preferred.
