Responsibilities:

Complete new resident interviews, data collection and move-in paperwork for admission.
Regular reporting and completion; includes bimonthly ISP, wellness visits and more
Monitor & communicate resident needs and and any changes
Develop continued knowledge of federal/ state & company regulations and policies.

Requirements:

Wisconsin Licensure in good standing
ASN or BSN.
Flexibility of schedule based on facility need
Must submit/ pass applicable pre-employment and annual tests
Meet all training & other applicable state/federal guideline
Minimum 1-2 years experience
CBRF and/ or RCAC nursing experience a plus
Medical records, QA, treatment plans and behavior analysis experience preferred

Qualities: Caring and compassionate, excellent interpersonal skills, problem solver with solid communication skills, responsible with choices and decisions made
