Registered Nurse (RN)
Long-Term Care / Rehabilitation
Position Summary
The Registered Nurse (RN) provides comprehensive nursing care to residents in a long-term care and rehabilitation setting. The RN is responsible for assessment, care planning, implementation, and evaluation of resident care, while supervising licensed and unlicensed staff to ensure safe, high-quality, patient-centered care.
Essential Duties & Responsibilities

Assess residents’ physical, psychosocial, and clinical needs and develop individualized care plans

Administer medications and treatments per physician orders and facility protocols

Monitor resident conditions, evaluate effectiveness of interventions, and initiate appropriate nursing actions

Coordinate and oversee care delivery by LPNs and CNAs
-
Perform and oversee wound care, IV therapy, and skilled nursing treatments as applicable
-
Communicate with physicians, interdisciplinary team members, residents, and families
-
Ensure accurate, timely documentation in the medical record
-
Participate in admissions, discharges, care conferences, and discharge planning
-
Respond to emergencies and provide clinical leadership during critical situations

Ensure compliance with infection control, safety standards, HIPAA, and regulatory requirements
Qualifications
-
Current RN license in good standing (state-approved)

Graduate of an accredited nursing program (ADN or BSN)

Long-term care or rehabilitation experience preferred

Strong leadership, clinical judgment, and communication skills
-
Ability to work flexible shifts including weekends and holidays
