(Archived) Temporary Population Health Outreach Specialist, Lowell, MA
Job Description
Seeking a Temporary Population Health Outreach Specialist to work with and be part of the Quality Department. This role is responsible for working with assigned primary care departments to focus on outreaching to patients that are overdue for cancer screenings, chronic disease appointments and labs, and well-child visits. The incumbent will work to ensure that the primary care departments have the support necessary to ensure patients are up to date on priority chronic disease and preventive care.
Education:
· High school diploma or GED required
· Proficiency in Microsoft Office (Excel, Word, Outlook, and PowerPoint) and online record keeping for work-based communication,
· Experience in community work, understanding of basic public health education concepts in health care strongly recommended
BILINGUAL SKILLS /LANGUAGES DESIRED
1. Portuguese/Khmer/Spanish/ Vietnamese fluency highly desirable.
