(Archived) Clinical Case Manager

Our mission is to provide accessible holistic patient centered care, to empower our community to achieve the highest quality health and well-being. We have an immediate need for a Clinical Case Manager, who is responsible for collaboration with clinical team members, patients and families to facilitate unmet health care needs within the context of the Patient Centered Medical Home (PCMH) model.

Qualifications:

1. Bachelor’s degree in social work is required.
2. 2 years of experience as a medical case manager is preferred.
3. Ability to read, comprehend and write instructions, correspondence and memos.
4. Fluency in both English/Spanish preferred
5. Ability to gather data in an organized fashion from varied sources.
6. Possession of a valid and unrestricted Arizona driver’s license
7. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
8. Must pass a substance abuse testing upon employment and submit to a random testing during the course of employment.
9. Must have an updated Immunization (IZ) Record.
10. Must submit a recent report of Tuberculosis (TB) skin test upon employment and provide a TB skin annually.
11. Must obtain a CPR Basic Life Support Provider Card within ninety (90) calendar days of employment and maintain a valid CPR card during employment.
