Summary

The clinic LPN supports the operations of a medical clinic and Medicare Advantage plan. The LPN functions as a Medical Concierge and is a key team member and provides high levels of clinical customer service to both the patient and his/her supervising Nurse Practitioner, while also driving quality outcomes for long-term care residents and Medicare Advantage plan members.

POSITION RESPONSIBILITIES:

LPN supports a full-time Nurse Practitioner in the daily operations of a primary care clinic embedded into a life plan community, while also supporting the clinical operations of a Medicare Advantage plan based on the same campus

Provides clinical services including vitals, basic phlebotomy, and immunization administration

Abstracts, interprets, and communicates reports and data from practice EMR and available reporting tools

Manages scheduling, intake, insurance coverage, copay collection, and understand other key components of front-office work, including Revenue Cycle and provider productivity

Preloads Patient Data into practice EMR from available sources, including life plan community records

Manages practice clinical inbox and correspondence, including encounter summaries, internal/external faxes, referrals, medical records requests, and payer communication

Participates in community care planning process

Serves as primary contact for clinic patients, Medicare Advantage plan members, and families/responsible parties, including the distribution/review of visit summaries. Goal is to provide high level of clinical customer service while attracting new patients and maintaining existing panel

POSITION REQUIREMENTS:

Minimum 2 years' experience in healthcare required, particularly in LPN work

Demonstrated ability to triage clinical needs and route to appropriate team member or care giver

EMR experience required

Primary Care or Long-Term Care work experience preferred

$25-28 per hour
