NP/PA is needed to join an outpatient post-acute care team in central California. Please see Job ID #50166.
- You will be working a Monday-Friday, 8 am-5 pm
- Focusing on patients recently discharged from the hospital
- Group is focused on helping patients transition from and staying out of the hospital
- Ideal candidate will have experience in internal medicine, hospitalist, geriatrics
- Please have a CA license in hand or well in the process
