Job Description
Experienced Nurse Practitioner or New Grad with SNF experience is needed for Nursing Home 1 hour outside of Plattsburgh, NY

- Full-time Permanent Position
- Schedule to be discussed with the provider
- The facility provides advanced skilled nursing and rehabilitative care while providing residents with a homelike atmosphere
- The provider will be conducting age-appropriate assessments of patients, including history taking and physical examination, Performs assessment/data collection in an on-going systematic manner, focusing on physiologic, psychosocial, cultural, spiritual, and cognitive status, patient/family education, family involvement, and patient advocacy
- A strong support staff of MD, APP and LPNs on site
- Open to New Gras with SNF experience as an RN
- NY license must be in process or in hand
