(Archived) Primary Care Nurse Practitioner

Our on-profit community health organization has opportunities for a full time Primary Care Nurse Practitioner to join our team in the county of Kaua’I, HW.

Requirements:

• Preferred: Experience in Medical Practice Management, Community based services and traditional healing specifically Native Hawaiian.
• Board Certified.
• Must be licensed to practice medicine and prescribe in the state of Hawaii.
• Must meet current CME requirements for licensure.
• Ability to work with multi-ethnic, multi-cultural and multi-lingual and low English proficiency patients.
• Participates or eligible to participate with the majority of health care plans we accepted.
• Applicants should have an unrestricted Hawai’i professional license and have completed training in a primary care specialty area.

The Primary Care Nurse Practitioner is under the direct supervision of the Medical Director. Our providers are participants in, and leaders of, the health care team which works to support the health and care of our patients.

We Provide services in primary health care, dental care, behavioral health counseling, substance abuse counseling, health promotion, health education, chronic disease management, nutritional counseling, fitness classes, outreach services, pharmacy services and case management services.
