Job Description

Patient Led, Clinician Operated healthcare clinic is seeking a like minded Nurse Practitioner with interest in dermatology to join on their journey. As a Nurse Practitioner, you will collaborate with other healthcare professionals to manage patient care and treatment, ensuring the highest standards of care are delivered.

Responsibilities:

Provide outstanding patient care by assessing and addressing the behavioral health needs of outpatient clients. Perform comprehensive health assessments and screenings for preventative measures. Actively manage common and complex medical conditions, implementing evidence-based treatment plans. Order, interpret, and evaluate diagnostic tests to diagnose and monitor patients' clinical problems and healthcare needs. Develop and document individualized care plans based on patient assessments and collaborate with the healthcare team to coordinate and optimize patient care. Clearly communicate medical procedures, treatment plans, and health education to patients and their families. Prescribe medication and other appropriate treatments to promote patient well-being. Participate in an on-call rotation as required, ensuring continuity of care for patients. Communicate assessment findings and collaborate with the healthcare team to identify and address any gaps in care. Qualifications: Minimum of 2 years of relevant clinical experience preferred; new graduates are also encouraged to apply. Possess an Advanced Practice License in Connecticut (or eligibility to apply for licensure). Hold a valid DEA license (or eligibility for registration). Master's degree in Nursing with current Nurse Practitioner certification in the specialty area (ANCC/AANP). Experience working with the geriatric population is preferred. Familiarity with Electronic Medical Record (EMR) systems, with eClinical Works experience preferred.Develop and document individualized care plans based on patient assessments and collaborate with the healthcare team to coordinate and optimize patient care.
