THIS POSITION IS A STATE TRAVELER POSITION-Based on the business needs, this position would require the ability to travel as needed for extended periods of time.
MUST HAVE A MINIMUM OF 1 YEAR NP EXPERIENCE.
May be required to travel to bordering states
M - F , No Call, and WORK / LIFE/BALANCE !
Perform Health Risk Assessments on geriatric patients in their homes, local nursing home, and skilled nursing facilities
See approximately 6 patients per day autonomously
Join a team of Nurse Practitioners whose collaborative care goal is to improve the healthcare of geriatrics, and reduce inappropriate hospitalizations

To join, you will need:
1 years Nurse Practitioner experience required
Current NP licensure in state of residence.
Board Certified Nurse Practitioner with specialty in Adult, Family or Geriatric Care
A dedication to delivering top notch patient care and education
Salary: $114,900.00 - $134,300.00 per year based on experience
