(Archived) Nurse Practitioner Assisted Living Facility
Job Description
Patient-Led, Clinician-Operated healthcare organization seeking a like-minded Nurse Practitioner to join us on our journey to provide exceptional care to our patients.
This is a Part-time position, up to 20 hours per week.
Compensation: $70 Per Hour
Key Responsibilities:
· Provide care to patients in assisted living facilities in collaboration with a physician and interdisciplinary.
· Conduct patient assessments, develop treatment plans, and ensure continuity of care.
· Administer follow-up appointments (2 hours per follow-up) and Annual Wellness Visits (1 hour per AWV).
· Collaborate with patients and families to provide patient-centered care.
· Maintain accurate and up-to-date patient records.
Qualifications:
· Nurse Practitioner (NP) certification.
· Current and unrestricted licensure to practice in the state of CT.
· Must have own transportation and ability to go to multiple sites
· Strong clinical and decision-making skills.
· Excellent communication and interpersonal abilities.
· Ability to work independently and manage patient care effectively.
Must be an independent and
highly motivated Nurse Practitioner.
