Job Description

We are seeking a dedicated Nurse Practitioner specializing in Gerontology to join our healthcare team. As a Nurse Practitioner in Gerontology, you will be responsible for providing primary health care to older adults in either a clinic or long-term care setting. Your role will involve performing selective medical services under the direction of clinic physicians, adhering to federal, state, and institutional guidelines and regulations governing long-term care facilities. Your primary objective will be to ensure the highest quality of care is maintained in alignment with the Clinic's Core Values and Promise. You will diagnose and manage acute episodic and chronic illnesses, prioritize health promotion and disease prevention, and deliver nursing and medical services to older adults in a clinic or nursing home environment.

Responsibilities:

  • Provide primary health care to older adults in a clinic or long-term care setting
  • Communicate clinical concerns about residents to designated primary care physicians and document all visits and interactions in the patient's chart, forwarding copies to the primary care physician for review
  • Order, conduct, and interpret diagnostic and laboratory tests, prescribe pharmacologic agents, and recommend nonpharmacologic therapies
  • Teach, counsel, and coordinate the care of older adults transitioning from community dwelling to inpatient care to long-term care
  • Collaborate with the Medical Director to implement resident care policies and coordinate essential medical consultation as needed
  • Make weekly visits to each facility as necessary, managing ongoing chronic conditions and addressing acute problems
  • Complete all HCFA mandated visits and attend quarterly policy meetings
  • Act as the primary contact for nursing home calls, taking call for all nursing home residents within the designated geographical area between 8:00 a.m. and 5:00 p.m., Monday through Friday
  • Participate as a team member in the provision of healthcare, collaborating with physicians, psychologists, therapists, dentists, dietitians, social workers, and administrative personnel to provide comprehensive care
  • See patients in area clinics if needed, when the nursing home patient load is not at maximum capacity

Qualifications:

  • Graduation from an accredited Nurse Practitioner program
  • Master's Degree from an accredited college or university
  • Current, unencumbered, active license to practice as an Advanced Registered Nurse Practitioner in the state of Iowa
  • Certification by a national organization as a Nurse Practitioner
  • Federal DEA and State Registration for prescriptive privileges
  • Certification as a Gerontological Nurse Practitioner (preferred)
  • Long-term care experience (preferred)
  • 1-2 years of experience as a Nurse Practitioner (preferred)
  • Maintain continuing education as required by state licensing and national certifying agencies
