Job Description

  • Work Dates Needed: Assignment: A-FOCHC-[Register to View] 9 July 15th - on going
  • Preferred Schedule: Monday-Friday 8-5
  • Worksite Setting: Outpatient
  • Scope of Work: 18 PPD, minimum is 18 PPD, mix of peds/adults, 1 RN & 2 MA's
  • Licenses, Certifications, Requirements: Active CA license, BLS
    • APCs with 3 years of experience in a similar setting
    • PAs must have a CA Controlled Substance Education Course Certificate
    • NPs must have a Furnishing License with Schedules II
    • Active DEA needed before the start
    • The following immunizations are required for credentialing (before the start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). Covid-19 Vaccine no longer required.
