Job Description

  • Work Dates Needed: As soon as credentialed for a 3-6 month contract with a possible extension
  • Preferred Schedule: Monday - Friday schedule | Tues, Thurs, Fri (8a-5p) and Mon and Wed (8a-7p)
  • Worksite Setting: Family Practice Outpatient clinic (FQHC)
  • Scope of Work: Family Practice seeing ages 2 and up. The provider will see a lot of Spanish, creole and Burmese language speaking patients, using a voice device that will serve as an interpreter. Census is 20 patients per day
  • Licenses, Certifications, Requirements: Active IN license, BLS, DEA, IN Controlled Substance Registration (CSR), and 2 years of experience.
  • EMR: Eclinicalworks
  • Pay Rate Preferred: $80-85/hr
