Job Description
Work Dates Needed: Starting August 3rd for a 3 month contract Preferred Schedule: Monday - Thursday 4-10's 8am - 6pm
Worksite Setting: Family practice FQHC, 1 other APP on site & 1 RN
Scope of Work: Full scope family medicine and primary care, seeing up to 24 scheduled patients per day, seeing all ages
Licenses, Certifications, Requirements: CA License, BLS, DEA, must have Medicare (PAVE), ORP (order, refer, prescribe), Partnership PHC (Medi-cal) minimum requirement
EMR: Ochin Epic
Pay Rate Preferred: $100 per hour. Accommodations covered.
