Job Description
- Work Dates Needed: ASAP - Ongoing
- Preferred Schedule: Mon-Fri (8a-5p)
Potential for FT/perm for interested providers - Worksite Setting: Outpatient
- Scope of Work: Annual wellness visits, hospital discharges, emergent patients, manage inbox and prescriptions, Avg 16-20 ppd, Age range: birth to geriatric
- Licenses, Certifications, Requirements: Active CT license, APRN, family medicine or primary care experience, experience seeing all patient ages, drive in candidates preferred, open to IM providers with experience
- EMR: Epic
- Pay Rate Preferred: $90/hr
