* Seeking an experienced advanced practice provider with a background in palliative care, hospice and /or geriatrics
* Serve as the primary point of contact for patients enrolled in the palliative care program
* Leverage the support of physicians, nurse care managers, behavioral health, social work, pharmacy and dieticians
* Establish and maintain relationships with patients, caregivers, legal representatives as well as PCP
* Perform initial assessments, develop patient management plans and coordinate communications as necessary
