Hospice and Palliative Care NP needed in Lancaster, NH

About the POSITION:

32 hours
· 16 hours at Home Health and Hospice in Berlin/Colebrook area
· 16 hours at Medical Center in Berlin/Colebrook/Lancaster

· high communication skills with patients and staff
· three or more years of experience working in an inpatient or nursing facility, internal medicine, or family practice environment is required
· The Nurse Practitioner will work in collaboration with the Interdisciplinary Team (IDT).
· will provide medical services necessary to meet the needs of palliative care and hospice patient through outpatient visits and in-home visits and will provide inpatient consults and be on rotation through Provider call schedule.
· Phone only, no need to come on site, one week per month, Sun-Sun. Clinician takes call and reaches out directly to APRN

About the COMP & BENEFITS:

Salaried Position. On-call is separate compensation. · CME time and money -up to $3,200 and one week (32 hours)
