Your New Role:
- Position: Nurse Practitioner or Physician Assistant
- Job Duration: Full Time/Permanent
- Employer: Multi-specialty Group
- Setting: Outpatient only
- Schedule: Mon – Fri; 8a – 5p with shared call
- Experience: 2+ year needed

Join this patient-centered care team committed to providing the highest quality end-of-life care with dignity, grace, comfort, and compassion supporting the patient's family and the patient. You can expect to work in a 100% outpatient practice, conduct home and face to face visits in Skilled Nursing Facilities and Assisted Living Facilities in neighboring communities. Mileage will be reimbursed. Working with a full support staff, ideal candidate needs to be a team player. Must have 2+ years of experience.
