Job Description
- Pay Rate: $65/hour + $33.77
- 1099 contract
- Place of Performance: Boxelder Job Corps Center, 22023 Job Corps Place, PO Box 110
Nemo, SD 57759
- 1 year with option to extend
- Work Schedule: Estimated Schedule: Approximately 1–2 days per week, depending on agreed shift lengths.
- Days & Hours: Mutually agreed upon between the provider and the center.
- Holidays & Closures: Scheduling adjustments coordinated with the center.
- After-Hours Calls: Not anticipated or required.
- Annual Commitment: Provider should be prepared to work the full 400 hours.

Clinical services as outlined in the Statement of Work (SOW), page 21 of the solicitation.

Required Qualifications & Documents
- Current resume/CV
- Valid, unrestricted medical license
- Valid DEA registration
- Medical professional liability insurance coverage of at least $1 million
