Job Description
- Setting: Direct Patient Care support in Adult Primary Care Clinic in Los Angeles for 2 providers
- Scope of practice: Direct Care to all patients in their panel overseeing clinical panel management for high risk patients in collaboration with PCMH team members.
- Dates: April, ongoing for 6 months with possible extension. LOCAL ONLY, no travel
- Work schedule: To be determined
- Support: Working with PCMH Team, familiar with ELM scorecard, ORCHID & eConsult
- EMR: Epic
- Requires: Active CA license, DEA, BLS
