Job Description
Work Dates Needed: 5/4 - 3 months or longer
Preferred Schedule: Monday - Friday 8am to 5pm
Worksite Setting: outpatient FQHC
Scope of Work: 18 patients a day, Must have prior experience and be comfortable treating children with BH (anxiety) issues Must have experience with Pediatric Chronic Care Management
Licenses, Certifications, Requirements: Board certified or BE 5 years out of residency, CA license, DEA, 3 years of FQHC experience, BLS, The following immunizations are required for credentialing (before the start date): Flu Vaccine, TB Status/X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), HCO & Vendor agree to a 30-day notice for ending all assignments.
