- Full-Time/Permanent Role
- 40hr week; exact hours will be discussed during the interview
- Travel required- Compensated
- Experience working with high-risk and medically complex patients with multiple comorbidities preferred.
- Multiple settings experience (urgent care, ER, inpatient, office, or community-based) preferred.
- Psychiatric and substance use disorder experience helpful- but not required
- Ideal candidate will have 2 plus years of direct experience
