Seeking an MDS Assessor to be responsible for coordinating assessments and completing MDS forms for all residents in accordance with State and Federal regulations and facility policies. This role involves obtaining necessary information, preparing schedules, inputting data, and ensuring accurate and timely transmission of MDS/PRI files to the Department of Health (DOH). Additionally, the MDS Assessor collaborates with the Quality Improvement Department and performs related duties.

Compensation: $110,000 – 122,000

Qualifications:

· Bachelor's Degree in Nursing.

· Minimum five (5) years of experience in clinical nursing.

· Two (2) years of experience in performing MDS completion.

· Licensed Registered Nurse with NYS.
