Matrix Providers is hiring a Registered Nurse (RN)Disease Mgmt to join our team of talented professionals who provide health care services to our Military Service Members and their families in Fairfield, CA.

  • Employment Status: Full Time
  • Compensation: Hourly position, paid bi-weekly, at a rate of $51.17 - $72.00 per hour.
  • Schedule: Monday – Friday, 7:30 AM - 4:30 PM.
  • Bonus: $500 bonus if you apply and complete onboarding docs within 72 hours. Sign-on and Relocation bonuses are available on a case-by-case basis. Ask your Recruiter!

Requirements: The Registered Nurse (RN) in Disease Mgmt must have and meet the following:

  • Degree: Associate's Degree in Nursing.
  • Education: Must be a graduate of a college or university accredited by the National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE)
  • Certification: Certification in disease-management-related areas such as Chronic Care Professional (CCP) by the Health Sciences Institute or Certified Health Coach (CHC) by the National Society of Health Coaches; or a patient education body such as Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators or Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board is highly desirable. It is highly recommended candidates have at least one of these certifications.
  • Experience: Six years of broad-based clinical nursing experience with three years of progressively increasing managed care responsibilities with a focus in Disease Management is highly recommended. Experience with disease management, clinical practice guidelines, performance improvement, and personnel management is strongly preferred. Full-time employment in a nursing field for at least 12 of the last 18 months is mandatory. Possess a thorough clinical knowledge and breadth of experience to be able to contribute to population-level assessment and management, identify appropriate use of CPGs, conduct patient self-care education, and track patient outcomes. Extensive knowledge of DM components: outcomes measurement, collaborative practice, CPGs, patient self–management, population identification processes, and process improvement.
  • Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse.

Job Summary:

  • Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in clinical care, quality, and cost-effectiveness
  • Ensure compliance with standards of care and practice by all established policies, procedures, and guidelines used in the Medical Treatment Facility (MTF).
  • Perform physical exams and health histories.
  • Provide health promotions, counseling, and education.
  • Administer medications, wound care, and numerous other personalized interventions.
  • Direct and supervise the care provided by other healthcare professionals.
  • Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
  • Recognize adverse signs and symptoms and quickly react in emergencies.
  • Communicate and collaborate with a diverse group of people to inform the healthcare team of plans/actions for teaching/education to benefit the patient/family and organization.
  • Make referral appointments and arrange specialty care as appropriate.
  • Perform nursing services as identified in the official Task Order.
  • Conduct research in support of improved practice and patient outcomes.
