Child and adolescent
20hrs/week - flexible which days
1 hour evaluation
15-30 minute medication management
If the provider is willing to travel to the Mesa location for 10 hrs per week would be a plus.
Ideally paneled with Mercycare and have AZ access ID ( not a deal breaker)
EMR- Profiler
Start ASAP
Credentialing average of 45 days
Commitment 3 months
