Job Description
Seeking a Community Health Worker for a diverse, community-based health care organization.
The CHW will to provide outreach education, distribute health promotion materials in English and Spanish. The CHW will conduct extensive outreach in their community, with the goal of enhancing awareness of resources, and to providers in order to increase their sensitivity to cultural health beliefs and to increase cross referrals. Collaboration with community agencies, coordinate resources/referrals and network with Community leaders. This position requires an individual committed to health equity and social justice, and adaptable in a variety of community settings.
Education:
- Bachelor’s Degree in Public Health, Health Education or related field, preferred.
- Community Health Worker Training Certificate, preferred.
- Two to four years’ experience in a community based setting or health related position.
- Must have reliable transportation.
- Bilingual in English, Spanish and or Khmer.
